She, along with one trusted office assistant who handles her voice messages are the only ones privy to your information. In fact your demographics and other personal information are privy only to Dr. Kurse. Your information will not be revealed to anyone except to whom you consent on your form or specifically request.
Your Rights as a Patient The practice of Psychiatry is regulated by the State of Colorado. You, as a client, are entitled to receive information about the methods of treatment, the duration of the treatment and the fee structure. You may seek a second opinion from another psychiatrist or may terminate your treatment at any time. The information provided by you is privileged information and will not be shared with anyone without your consent, except for certain legal exceptions (Duty to inform when someone is at risk of harm). That is determined by the provider and will be discussed with you.
Protected Health Information My "Protected Health Information" means health information, including my demographic information, collected from me and created or received by my physician, another health care provider, a health plan, my employer or a health care clearinghouse. This protected health information relates to my past, present or future physical or mental health or condition and identifies me, or there is a reasonable basis to believe the information may identify me.
